How it Works

  1. Patient receives lab order from their provider.
  2. Lab order should be emailed to Kat@loyaltymobilephlebotomy.com for accurate pricing.
  3. Patient can then set up an appointment  online or via telephone
  4. Phlebotomist comes to you at your set time, date, and location
  5. Phlebotomist will then collect all required specimens, your lab slip, and your insurance info
  6. Drops off samples, lab slip, and insurance info to the lab
  7. Doctor will receive results and deliver them to patient

Payment

Loyalty Mobile Phlebotomy is a private pay company.

We currently DO NOT accept insurance.


Payments that have not been prepaid, will be due at the time of service.

  • Cancellation Policy

    A $35.00 deposit is required when booking an appointment that will go towards your total.


    • Cancellations made 48 hours before scheduled appointment time will get their deposit refunded
    • Cancellations made 24 hours before scheduled appointment time, you lose your deposit and will need to give a new deposit to rebook.
    • Any NO SHOW/NO CALL will be charged the FULL amount of service
